Discussing the international telemedicine market #ATA2012

Here at the American Telemedicine Association (ATA) 2012 annual conference, there are a number of discussions revolving around the international application of telemedicine. This afternoon, I attended the Industry Executive Panel 5: International Markets. What does the globalization of telemedicine mean for the industry? Participants included: AMD Global Telemedicine, Sun Pa, IBM Global Business Services, and Chorus Call.

A growing number of countries are using telemedicine to delivery care in areas that currently lack access. Hence, telemedicine is bridging a crucial gap to access, but there are regulatory challenges that telemedicine  companies must understand that are unique to each country. Also, cultural factors must be addressed when telemedicine programs are being developed. Developing nations have needs and opportunities that are being addressed through the use of low-cost strategies that leverage mobile devices and ubiquitous wireless networks. As telemedicine solutions integrate with various electronic health records (EHRs), the issue of interoperability becomes another challenge that we must be ready to address. Right now, there are no international standards that are universally being applied to health information. So, global health information exchange is a significant challenge right now. Another major challenge surrounds the issue of sustainability. Who is paying for the telemedicine services in different countries? Does the government cover these types of health care costs? Or, are individual patients paying?

What is the Telemedicine Market? Industry Executive Panel Discussion at #ATA2012

This morning, I am at the American Telemedicine Association 2012 annual conference and I'm listening to the Industry Executive Panel 1 titled, "The Telemedicine Market." Whose numbers are right? What are the growth sectors and who are our customers? Panel members were from Cisco, Bosch, Vidyo, and HP.

The world of telemedicine is very broad and it includes direct patient care (consultations, referrals) to even the delivery of medical education though the use of telecommunications technologies. There are many ways to define/segment the telemedicine market because of the wide breath of applications. We can conservatively view telemedicine as a $10 billion market. The problems right now surround technology adoption and sustainable business models.

We are in a major transition period right now and we are seeing some major challenges with adoption. Plus, the rules, regulations, and opportunities in every country varies around the application of telemedicine. You have to select the right patients who will benefit from telemedicine/telehealth. Providers and patients need to be comfortable interacting in a "virtual" environment. This will bring about improved analytics, engagement, and patient care. Telecommunications tools must get integrated into the standard clinical workflow so that they become a natural extension of health care delivery.

Here in the United States, consumer trends are also significantly impacting the telemedicine market. Patients are becoming more empowered to participate in their own health care. They are effectively "shopping around" for medical services. In many other parts of the world, the delivery of health care is so different that the application of telemedicine is a different discussion. Here in the US, roughly 20 to 40% of our medical encounters with patients could be delivered through telemedicine. Imagine what would happen to office visits if we switched the encounter to webcam + computer screen.

Technology Continues to Fight AIDS

In 2008, Sub-Saharan Africa was populated with over 22 million HIV+ inhabitants, and currently there are over 5 million South Africans infected with the virus. Worldwide, there are upwards of 40 million people infected with HIV, a very frightening number. But with the coming of the 22nd annual World AIDS Day, it’s important to take note the progress that has been made in the fight against HIV/AIDS. At the same time, it’s very vital we familiarize ourselves with a couple great HIV research and technology investors.

Granted, there have already been major advances concerning affordable microbicides and vaccines as preventative measures against the virus. Similarly, the introduction of low-cost antiretroviral drugs has allowed people already infected to lead longer, healthier and happy lives.

This can most certainly be attributed to tremendous associations like the CGI (Clinton Global Initiative). The Clinton Global Initiative has put a tremendous amount of money into AIDS research. Known for his work in raising money for Hurricane/Tsnuami victims, former President Clinton and his close personal aide Doug Band also have great interest in tackling one of the deadliest STDs in the world, HIV/AIDS. Back in 2006, Clinton helped open people’s eyes to the severity of the disease in foreign states by traveling deep into Burma with the crew of 60 Minutes.

I heard a great example today of how the HP Digital Hospital Infrastructure can be used to automate several critical steps in the clinical workflow when an OB/GYN prepares a patient for emergent C-section by pushing a single button. If you haven't heard of the HP Digital Hospital Infrastructure, here's a brief description from the HP website:

In a digital hospital, systems and people communicate real time information, improving responsiveness, accuracy and ultimately patient care. Healthcare professionals can receive messages and alerts on their wireless devices about new lab results, outcomes of radiology exams, patient monitoring information, and the location of critical equipment. Lunch with Sermo's Dr. Daniel Palestrant here at #HIMSS10

I had the pleasure of having lunch with Dr. Daniel Palestrant (Founder and CEO of Sermo) today here at HIMSS10. We had a great discussions today and we talked about physician adoption of EHRs, the use of social media, the evolution of Sermo, our families, and many other interesting health IT topics. Given that the vast majority of physicians are not early adopters of technology, what has made Sermo so successful as an online physician community? It was a really fun discussion given my personal interests in social media and health information technology.

Yesterday, athenahealth and Sermo released data about "Enabling Physicians to Leverage Technology to Improve Patient Care." What do physicians really think about ARRA and HITECH? What are their opinions regarding EHRs and "meaningful use"? Are physician opinions polarized, or are they in unison on these topics? Let's wait to see what type of data athenahealth and Sermo release over the next several weeks.

Daniel shared some great insights about the practical needs of physicians as they relate to practice management, billing, clinical workflow, reimbursement, EHR usability, and more. Through Daniel, I also met Adam Sharp, MD, Chief Medical Officer of Sermo and Jonathan Bush, athenahealth's Chairman and CEO. Thanks again for your time today Daniel. Confusing IT terms

I was reading through an e-newsletter sent by HP (yes, I do read these) and I was amused by an article about baffling IT (information technology) terms. If you work in health IT, then you're probably very familiar with these terms. However, if you're not very "tech savvy," then you might get enlightened here. Let me highlight a few of these confusing IT terms:

• VoIP: A T-Mobile survey found that one in ten respondents thought VoIP can improve visibility for the visually impaired! It actually stands for Voice over Internet Protocol, and literally refers to the ability to transmit the voice over the Internet.
• ASP: This one is particularly tricky because it’s an acronym with two meanings. An Active Server Page is an HTML page that includes one or more scripts (small embedded programs) that are processed on a Microsoft® Web server before the page is sent to the user. An Application Service Provider is a business that provides computer-based services to customers over a network.
• Petaflop: This is a word you’ll probably see in conjunction with discussions of supercomputing. A petaflop can be expressed as a thousand trillion floating point operations per second; it is a measure of performance for the fastest computers in the world.

Touch Computing Helping Kids with Autism

Kids with autism typically struggle in three areas: socialization, communication, and behavior. Touch-screen computer technology may be instrumental in helping kids with speech. NBC Bay Area News reports that Hope Technology school in Palo Alto California is using HP TouchSmart PCs to help children with autism. Kids are touching the screen to communicate short messages. Those machines were donated to the school by HP when executives found out that the school had lost all its PCs to theft. Now, these touch-screen computers are helping autistic children communicate. Who needs keyboards when you can simply touch the screen? I'm predicting that future computers will be primarily driven by voice recognition and touch screen. The keyboard will be a thing of the past.

Multi-Touch Screens on a Tablet PC

The phrase "multi-touch" has gotten confusing in the tablet PC industry. My Lenovo Thinkpad X200 tablet is considered to have a multi-touch screen because I can use my active digitizer pen or my finger as an input device. However, I can't use two fingers on the screen because it is a passive screen (not a capacitive touch screen like the iPhone or BlackBerry Storm).

I was walking through Best Buy today and I played with the convertible tablet PC made by HP. It was the HP tablet TX2Z that features a capacitive multi-touch display (which means that you can use two fingers on the screen and perform gestures).

Windows 7 will include the capability to use some of those cool multi-touch gestures. Apple already leverages this technology on the iPhone and on their MacBooks.
